Parent or Guardian Consent Agreement

A parent or legal guardian must review and accept this agreement before an applicant under 18 years of age may become a member of the Tri County R/C Club or participate in club activities.

By accepting this agreement, I confirm that I am the parent or legal guardian of the minor included in this membership application. I give permission for the minor to participate in Tri County R/C Club activities, including flight instruction, radio-controlled aircraft operations, club events, field activities, meetings, demonstrations, and other related activities.

Understanding the Risks

I understand that operating and being near radio-controlled aircraft involves inherent risks. These risks may include, but are not limited to:

• Loss of aircraft control
• Equipment, engine, motor, radio, or battery failure
• Propeller or rotor injuries
• Aircraft collisions
• Hard landings and crashes
• Battery fires or charging accidents
• Contact with tools or equipment
• Uneven ground, falls, weather conditions, and other field hazards
• Damage to aircraft, vehicles, equipment, or other property
• Serious bodily injury, permanent disability, or death

I understand that some risks are a natural part of radio-controlled aircraft activities and cannot be completely avoided or eliminated, even when reasonable care, established safety procedures, and proper supervision are used.

Club Rules and Safety Requirements

I understand and agree that:

1. The minor must follow all Tri County R/C Club bylaws, field rules, safety standards, membership agreements, and instructions provided by club officers, flight instructors, safety officers, and other authorized club representatives.
2. The minor must comply with all applicable Academy of Model Aeronautics and Federal Aviation Administration requirements.
3. The minor must complete any required registrations, safety tests, training, or other requirements before operating an aircraft.
4. All aircraft and related equipment used by the minor must be properly maintained, inspected before operation, and considered safe to fly.
5. The minor may be required to fly under the supervision of an approved flight instructor or responsible adult until the club determines that the minor can safely operate an aircraft.
6. A parent, legal guardian, or responsible adult designated by the parent or guardian must remain present whenever required by club policy or club leadership.
7. The minor must immediately follow instructions to stop flying or leave the flight area when directed because of unsafe conduct, equipment concerns, weather conditions, field conditions, or other safety concerns.
8. The minor must treat club members, guests, instructors, officers, volunteers, and other participants with respect. Unsafe, threatening, disruptive, or inappropriate behavior may result in suspension or termination of club privileges.
9. The minor may not operate an FPV aircraft without an appropriate spotter and must follow all club requirements for FPV operations.
10. The parent or guardian is responsible for explaining the club’s rules and safety requirements to the minor in a manner the minor can understand.

Flight Instruction and Voluntary Assistance

Flight instructors and other club members may voluntarily provide instruction, assistance, equipment checks, recommendations, or other help to the minor.

I understand that flight instruction does not guarantee that an accident, hard landing, crash, equipment failure, injury, or property damage will not occur. Instructors and other members are not responsible for aircraft damage or other mishaps that may occur while voluntarily providing instruction or assistance, to the fullest extent permitted by law.

I understand that the parent or guardian remains responsible for determining whether the minor’s aircraft, equipment, abilities, and level of supervision are appropriate for participation.

Assumption of Risk

I knowingly and voluntarily accept and assume, on behalf of myself and the minor applicant, all known and unknown inherent risks associated with radio-controlled aircraft activities and the minor’s presence at the club’s flying field, meetings, events, and other activities.

I understand that participation is voluntary and that I may choose not to allow the minor to participate.

Release of Liability

To the fullest extent permitted by law, I agree that the Tri County R/C Club will not be responsible or liable for any injury, illness, disability, death, accident, property damage, aircraft damage, equipment damage, theft, financial loss, or other damages arising from or connected with:

• The minor’s participation in club activities
• The minor’s operation of radio-controlled aircraft
• The minor’s presence at the flying field or a club event
• The actions of another pilot, participant, member, guest, or spectator
• Flight instruction or voluntary assistance provided by club members
• Equipment failure, loss of aircraft control, crashes, or hard landings
• Field, weather, environmental, or other conditions
• The inherent and ordinary risks associated with model aviation

On behalf of myself and the minor applicant, I release and hold harmless the Tri County R/C Club and its officers, directors, flight instructors, safety officers, members, volunteers, representatives, and agents from claims arising from the inherent risks and ordinary negligence associated with the minor’s participation in club activities, except for claims that cannot legally be released.

This agreement does not release any person or organization from liability for gross negligence, intentional misconduct, or any other conduct that cannot be released under applicable law.

Aircraft, Equipment, and Personal Property

I understand that the Tri County R/C Club does not guarantee the safety, reliability, performance, or condition of any aircraft, transmitter, receiver, motor, engine, battery, charger, tool, accessory, or other equipment.

The club is not responsible for the loss, theft, destruction, or damage of the minor’s aircraft, equipment, vehicles, or personal property.

The parent or guardian is responsible for ensuring that the minor uses appropriate aircraft and equipment and that all equipment is properly maintained and operated.

Emergency Assistance

If an emergency occurs and the parent, guardian, or designated responsible adult cannot be immediately reached, I authorize club representatives to contact emergency services and provide or obtain reasonable emergency assistance for the minor.

I understand that club members are not medical professionals and are not required to provide medical treatment beyond contacting emergency services or providing reasonable assistance within their abilities.

I understand that the Tri County R/C Club does not provide medical or accident insurance for the minor. The parent or guardian remains responsible for medical expenses, transportation expenses, property damage, and other costs resulting from the minor’s participation.

Acknowledgment of Club Documents

I confirm that I have received, reviewed, or been given access to the following:

• Tri County R/C Club bylaws
• Club safety standards and field rules
• Preliminary Agreement
• Membership requirements
• Parent or Guardian Consent Agreement
• Applicable Academy of Model Aeronautics requirements
• Applicable Federal Aviation Administration requirements

Club documents are available at tricountyrcdunnellonflorida.com or may be requested from a club officer.

Parent or Guardian Acknowledgment

By selecting the agreement checkbox and submitting the membership application, I confirm that:

• I am the parent or legal guardian of the minor applicant.
• I have read and understand this Parent or Guardian Consent Agreement.
• I understand that radio-controlled aircraft activities may result in serious injury, death, or property damage.
• I voluntarily accept and assume the risks associated with the minor’s participation.
• I give permission for the minor to participate in Tri County R/C Club activities.
• I agree that the minor will follow all applicable club rules and safety requirements.
• I accept the Release of Liability contained in this agreement.
• The information provided in the membership application is complete and accurate.
• My electronic acceptance has the same effect as my handwritten signature.
